Online Application Instructions

1. Go to the Montana Department of Revenue TAP Portal website: https://tap.dor.mt.gov/ . Ignore the login prompt and scroll down until you find the cannabis plant.

2. Click on Apply for or Manage a Medical Marijuana Card

3. You will now choose application type.  Choose new patient application if this is your first card or if your old card is >30 days expired.  Choose renewal application if your card is current or >30 days expired.  Choose adult application on the next page.  If you are renewing, it will ask for your 6-digit card number so find that now. 

4. This next screen asks for Cardholder Information (You).

  • First and Last Name

  • Date of Birth

  • Social Security Number

  • Phone Number

  • Email Address

5. The next section is the “Address Section”. Enter your home physical address as the location address.  Click on the blue button that says Address needs to be verified.  Verifying just converts it to a postal friendly format and some addresses will not verify.  This usually happens with new homes or remote/unusual addresses.  It is fine to proceed without a verified address.  If your mailing address is not the same as your home address, you will need to select NO and then enter your mailing address. 

6. The next section is “Designated Purchaser(s)”. A cardholder can designate up to 2 other individuals to purchase their medical marijuana for them. The designated purchaser(s) must be at least 21 years old.  If you select YES, you will be required to upload a copy of their ID/DL later in the application. 

7. The next section is “Physician Information”. Enter the physician’s info listed on your physician statement that you received during your appointment.  Click on the red link Address needs to be verified to verify the office address.  If you have a petition from your physician for a monthly limit increase, select YES. You will be required to upload the limit increase documentation later. 

8. The next section is “Debilitating Conditions”.  You must select each condition marked by the physician on page 2 of your physician statement. 

9. The next section is “Cultivation Questions”.  Select YES if you will be growing for yourself or NO if you will not. If you select YES, you will need to indicate if you own the location that you intend to grow at. If you own the location, you will be required to provide proof of ownership (homeowners insurance, property tax bill, etc.).  If you do not own the location, you will need the property owner to fill out a landlord permission.

10. The next section is the “Divert Statement”. If approved for a medical marijuana card, you and your designated purchaser(s), if applicable, are agreeing not to divert any form of medical marijuana to any other person.

11. You will upload the documentation required by the state next. Required attachments will vary depending upon the applicant’s selections during the application process. The application will require you to give a name to each attachment that you upload.  You can name the uploads anything you choose, but each one must be unique. 

These attachments are required of all applicants:

  • Photo of Applicant - This is self-portrait with plain background, no hat or glasses on your head. Include some space above your head and the top of your shoulders in the photo to allow for some cropping.

  • Physician Statement – 3-page form completed and signed by the doctor

  • Documentation of Montana Residency – MT ID/Driver’s License, Tribal ID, Resident Fishing/Hunting License, or Montana Voter Registration Card

The following attachments are only required if you have a limit increase, list someone as your designated purchaser, or grow for yourself. 

  • Limit Increase (if applicable)

  • Official Documentation of Designated Purchaser(s) Date of Birth (if applicable)

  • Proof of Property Ownership is required if you are a home grower AND own the grow location.

  • Notarized Landlord Permission Form if you are a home grower that DOESN’T own the grow location

12. On the next page, select NO to paying by e-check to make payment via credit or debit card. Payment can be submitted electronically by e-check, by mail, or in-person after submission, but credit/debit card is the easiest and quickest method of payment.

13. On the next page, you are going to type your name to swear and affirm that all the information you have provided is true.

14. On the next page, you will have the opportunity to download and print your temporary card.  Once you have done that, click on Return Now next to the countdown clock. 

15. It will now ask for “Contact Information” (that is you).  Enter your name, phone number, and email twice.  Answer YES to the Taxpayer and Contact being the Same.  Answer NO to receiving a letter or billAnswer NO to submitting a payment for an Unclaimed Property Holder.  Answer NO to knowing the Account ID.  Now select SSN and enter your number.  Select Medical Marijuana Card and Permit from the dropdown menu.  The application fee is $20 + processing fee ($21.77) and is non-refundable regardless of application approval or denial. 

16. The Payment page is next.  Fill in the required fields and then follow the rest of screen prompts to complete your payment.  An email receipt will be sent to you if your email address was provided.    

 

If there is an error on your application, the state will email you a notification detailing what needs to be corrected.  You will have 30 days to correct the error.  If you do not fix it within the 30 days, you will have to resubmit a new application and pay the fee again.
